The Palliative Care Department of Bordeaux University Hospital (CHU de Bordeaux) provides specialist inpatient palliative care, including two dedicated palliative care units, an inpatient support team, outpatient clinics, and support to community-based teams. The service delivers complex symptom management, anticipatory care planning, psychosocial support, and coordination especially with oncology, geriatrics, internal medicine, and primary care. The department also leads educational activities for medical and nursing staff and participates in national and European research programmes focused on complexity, early identification, and care coordination.
SPICT Uses
SPICT is being implemented as a routine clinical tool to help identify patients who may benefit from a palliative care approach across the hospital. It is used within the palliative care unit, during multidisciplinary reviews, and in support-team assessments. The tool is also being disseminated and taught to other hospital departments to harmonise early identification practices and improve referral pathways.
In research, SPICT will be used as a standardised screening instrument to support inclusion procedures in studies focusing on palliative care needs.

Multiprofessional Primary Health Care Center "The Vines of the Abbey" - Saint Just en Chaussée

La Maison de Santé Pluriprofessionnelle “Les Vignes de l’Abbaye” (MSP VA) regroupe 17 professionnels de santé (7 médecins généralistes, 4 infirmières, 2 kinésithérapeutes, 1 sage-femme, 1 psychologue, 1 conseillère parentale, 1 diététicienne) sur un territoire de santé rural, en Picardie. L’équipe de soins primaires de la MSP VA a développé un projet interprofessionnel* visant à promouvoir la santé et la volonté des patients se rapprochant de la fin de leur vie, et à faciliter, le cas échéant, leur accès à des ressources spécialisées de soins palliatifs.
The Multiprofessional Primary Health Care Center “The Vines of the Abbey” (MPHC VA) consists of 17 health professionals (7 GPs, 4 nurses, 2 physiotherapists, 1 midwife, 1 psychologist, 1 parental counselor, 1 dietetician) brought together on a rural health care territory in Picardy. The primary care team has developed an interprofessional project* aiming to promoting the health and will of patients approaching the end of their life, and to facilitating, if appropriate, their access to specialist resources in palliative care.

SPICT Uses
Le projet SCoP3 repose sur trois principes : 1/ identifier les patients se rapprochant de la fin de leur vie dans le cadre des soins primaires 2/ évaluation multidimensionnelle de leurs besoins palliatifs 3/ planification anticipée des soins.
Chaque membre de l’équipe de soins primaires (MG, infirmières, kinés) utilise le SPICT pour identifier les personnes risquant de décéder, ou dont la santé est en voie de détérioration. Une fois qu’un patient a été identifié, son dossier est inscrit sur le registre SCoP3 de la MSP VA afin d’être régulièrement discuté par l’équipe de soins primaires, durant les réunions de coordination interprofessionnelle. L’utilisation de SPICT permet également de rappeler aux professionnels les différentes circonstances où initier la planification anticipée des soins avec les patients.
SCoP3 project relies on three main principles:
- identifying patients approaching the end of their life in the community
- multidimensional assessment of their palliative needs
- anticipatory care planning.
Each member of the primary care team (GPs, nurses, physios) uses the SPICT to identifying people at risk of deteriorating and dying. Once a patient has been identified, his/her record is included in the SCoP3 register of the MPHC VA, and is regularly discussed by the primary care team during interprofessional coordination meetings. Using SPICT is also helpful in reminding health care professionals of the various circumstances where to initiate anticipatory care planning with patients.
