Advance Care Planning and supporting clinicians (nursing and medical) to ensure adult patients have the opportunity to express their end of life wishes and preferences irrespective of diagnosis.

To support nursing and medical teams to prognosticate and identify people who may be in the last year of life in order to offer more opportunities for important conversations before a crisis. We use the tool directly and within our education of health and social care professionals.

We are a small team within Cwm Taf UHB who support patients and professionals by having conversations, education and raising professional and public awarenesses.
